After you fill out the menu you can email, fax (218-365-3690) or mail the completed form or give us a call & we can fill out the menu with you. 

Emailing Form:
After you are done filling out the menu hit the 'Submit by email' button on the top of the form and you will see the following on the email:
"The attached file contains data that was entered into a form. It is not the form itself."

      Example: 2 gluten free & 2 no alergy - 2 will get gluten free items & 2 will get regular choices.
